Changes at The Greenhouse – July 2018

Dear Patients,

Please find below some information about the GP services at The Greenhouse and an invitation for you to give your views on how these could be provided in future. There is a survey also to enable you to feedback your experience at the surgery.

What is happening at my surgery?

The contract to run your GP surgery is coming to an end and we need to put in place another contract to ensure that services continue. Since the NHS was founded in 1948, GP Practices have been independently run businesses which are contracted by the NHS to provide medical services for patients. These businesses can be owned and run by an individual; a company; a partnership; or a social enterprise. The organisation that manages your GP practice employs the doctors, nurses and other staff who work there.

GP services at The Greenhouse are currently managed by AT Medics. In 2016, AT Medics was given a caretaking contract to run those services. This contract will come to an end as planned on 31st March 2019 and a new contract for GP services will be put in place on 01 April 2019. The new contract may or may not be with the current organisation.

This does not mean that your practice will close or that you will need to find a new doctor. NHS England and City and Hackney Clinical Commissioning Group (CCG) will make sure that your GP services continue uninterrupted.

We will do this by running a formal selection or ‘tendering’ process, which will result in awarding either a new organisation or the existing organisation with a contract to manage your surgery for the next 5 to 10 years. As part of this, we will be asking patients and others for their views on the services currently provided.

It is a legal requirement that we run this selection process in a fair and transparent way, and we want to ensure that any decisions made are in the best interests of patients, staff and taxpayers.

What does this mean for patients?

You do not need to do anything. GP services will continue to be provided at The Greenhouse in the period leading up to, during and after this process.

Although it is possible there may be some changes to staff, we expect a number of the current doctors, nurses and receptionists to continue working in the practice.

Getting involved

NHS England and City and Hackney CCG want to hear from patients to make sure that your views are taken into account in making this important decision about who manages GP services in your surgery.

You can also attend the information session for The Greenhouse where you can and put forward ideas and suggestions to improve the services at your surgery: Tuesday 17th July 2018 between 11:00 to 12:30 at The Greenhouse.

Printed copies of responses to frequently asked questions, will be made available at the surgery for your information.

FAQs

What is happening at my GP surgery?

GP Practices aren’t managed directly by the NHS. Instead, NHS England appoints an organisation to manage your GP practice. This can be run by a single GP; a group of GPs; a social enterprise or a private company. The organisation that manages your GP practice employs the doctors, nurses and other staff who work there.

The contract with the current provider of GP services at your GP practice is due to end on 31st March 2019. As a result, NHS England and City and Hackney Clinical Commissioning Group (CCG) are required to review the contract and put it out to a competitive process where a variety of providers (including AT Medic who currently supplies your GP service) can compete to run the surgery

We will make sure that your GP services continue uninterrupted and you don’t need to do anything. However, if you would like to share your views on the services that you think should be provided by your GP practice we would love to hear from you.

Why can’t my existing doctors continue to run the practice?

The contract to run the surgery covers a set period of time. This enables NHS England and the CCG to periodically check that the organisation appointed to provide services remains the best option for patients. The current contract is due to expire on 31st March 2019.

AT Medics is aware of this and is encouraged to apply, through the formal selection process, to run the surgery again. The law says that we must treat AT Medics in the same way as any other GPs or organisations that may be interested in running the practice.

How can I get involved?

Patients can share their views on their surgery in a number of ways:

This is an opportunity for you to let us know what you think needs to be improved as well as what currently works well. Your views will help us to design a service that meets the needs of patients.

What arrangements are being made for patients whilst the procurement process is underway?

GP services for patients will continue before, during, and after this process and there is nothing you need to do.

If a different organisation is appointed will our surgery close?

No. The Greenhouse will continue to provide GP services without a break.

What will happen to the doctors and practice staff?

Whatever the outcome of the procurement process, regulations exist to protect the employment of existing staff. This includes any employed doctors, nurses, receptionists and administration staff. As a result, even if a new provider is chosen to provide the GP service, we would expect many of the doctors, nurses and other practice staff to remain at the surgery.

Will there be any changes to the services offered at The Greenhouse?

We will be seeking to make some changes to the services provided. We will take into account patients’ views collected through the questionnaire advised in Q3 above. Asking for patients’ views on the GP service that they currently receive is an important part of the process of putting together the new contract. We will look at the feedback received through the questionnaires and the Patient Event to take account of this.

You will be kept fully informed of any changes that will be made.

Do I need to do anything?

You do not need to do anything. GP services will continue to be provided at The Greenhouse in the period leading up to, during and after this procurement process.

If I want to register at another practice, do I need to do this myself?

We hope that you will be happy to remain at The Greenhouse but if you wish to change GP practice at any time you should contact the surgery of your choice and ask them to register you. You can find a surgery near you at My Health London. It has information about practices, including opening hours and patient satisfaction survey results.

What are NHS England and Clinical Commissioning Groups (CCGs)?
NHS England is the national body responsible for the establishment and maintenance of contracts with GPs throughout the whole of England. It took over the responsibility from Primary Care Trusts on the 1st April 2013. NHS England is divided into a number of Regional Teams that are responsible for geographical areas. Your practice falls under the responsibility of the London Regional Team of NHS England.

Clinical Commissioning Groups (CCGs) were created following the Health and Social Care Act in 2012, and replaced Primary Care Trusts on 1 April 2013. They are clinically-led statutory NHS bodies responsible for the planning and commissioning of health care services for their local area. There are now 207 CCGs in England. The Greenhouse is part of City and Hackney Clinical Commissioning Group (CCG).
